Prävention Antibiotika-resistenter Erreger

Antibiotika sind essentiell, um sowohl in der Human- als auch in der Veterinärmedizin bakterielle Infektionen wirksam behandeln zu können. Im Sinne des „One Health“-Konzeptes wurden deshalb im Forschungsverbund #1Health-PREVENT Erreger, die gegen viele klinisch wichtige Antibiotika unempfindlich geworden sind, erforscht. Dabei wurden epidemiologische Studien zur Verbreitung von solchen multiresistenten Erregern (MRE) durchgeführt und Interventionen validiert, die einer Selektion von MRE vorbeugen bzw. eine Übertragung von MRE zwischen Tier und Mensch verhindern. Die Arbeiten erfolgten in enger Kooperation mit anderen Verbünden und Projekten des Öffentlichen Gesundheitsdienstes (ÖGD) innerhalb des Forschungsnetzwerks Zoonotische Infektionskrankheiten.

AP 1: #1Health-Epidemiologie:

Der Forschungsverbund hat vor allem die Verbreitung von Carbapenem- (CRE) und Colistin-resistenten Enterobakterien (Col-E), multiresistenten Koagulase-negativen Staphylokokken (KoNS), Methicillin-resistenten Staphylococcus aureus (MRSA)-Stämmen, sowie Biozid-unempfindlichen Erregern in der Schweine-, Rinder- und Geflügelhaltung, aber auch bei Haustieren, Pferden und bei direkt exponierten Menschen untersucht.

Dabei wurde gezeigt, dass CRE in der Schweine- und Geflügelhaltung in Deutschland nicht häufig vorkommen, dass jedoch Col-E sowohl in Schweine- und Geflügelhaltungen (auch bei den dort tätigen Mitarbeitenden) gefunden werden. Auch MRSA lässt sich bereits nach kurzer Aufenthaltsdauer in Nasenabstrichen von Schweinestall-Besuchern nachweisen, die Besiedlung ist jedoch temporär. Zudem wurde gezeigt, dass KoNS aus Schweine- und Rinderhaltungen oft multiresistent sind, wobei auch Unempfindlichkeiten gegenüber für den Menschen wichtigen Reserveantibiotika wie Daptomycin und Linezolid vorkommen. Viele der gefundenen Antibiotika-Resistenzgene liegen auf übertragbaren genetischen Elementen, welche die Verbreitung dieser Resistenzen fördern könnten.

Als Voraussetzung für die Biozid-Empfindlichkeitsprüfung von Bakterien wurden eine Testmethode mittels Bouillon-Mikrodilution und Qualitätskontrollbereiche für definierte Referenzstämme (S. aureus, Enterococcus hirae, Escherichia coli, Pseudomonas aeruginosa) und Biozide (Benzalkoniumchlorid, Chlorhexidin, Octenidin und Polyhexanid) erarbeitet wobei erste Untersuchungen von Zoonose-Erregern unimodale Verteilungen zeigten

Weitere epidemiologische Studien fanden, dass S. aureus in 19% der Haushalte in denen Menschen nasal besiedelt waren, molekulargenetisch dieselben S. aureus auch Hunde des Haushalts kolonisierten, während Übertragungen des bei Hunden verbreiteten Erregers Staphylococcus pseudintermedius (37.5% der Hunde besiedelt), nur selten vorkommt (0,6% der Haushaltskontaktpersonen).

AP 2: #1Health-Interventionen:

Der Forschungsverbund hat Interventionen in schweinehaltenden Betrieben durchgeführt mit dem Ziel zu untersuchen, ob sich durch alternative Haltungsverfahren (z.B. auf Stroh), das Vorkommen von MRE reduzieren lässt. Dabei wurde gezeigt, dass Strohhaltung die MRSA-Besiedlung von Schweinen deutlich reduziert bzw. bis Mastende die Tiere vollkommen von MRSA befreit sind. Dies geht mit einer signifikanten Diversitätserhöhung im nasalen Mikrobiom einher, wodurch es vermutlich zu kompetitiven Effekten kommt. Ebenso konnte in diesem Zusammenhang gezeigt werden, dass in Betrieben mit Strohhaltung sich auch die Last mit multiresistenten KoNS verringert.

In Milchviehbetrieben wurden anhand epidemiologischer Untersuchungen zur Verbreitung von MRSA Schwachstellen erkannt, welche die Verbreitung von MRSA innerhalb der Herde sowie von Kühen auf Kälber bzw. eine Kontamination von Rohmilch ermöglichen. Es wurden Interventionsmaßnahmen entwickelt, um das MRSA-Vorkommen in den Milchviehbetrieben zu reduzieren. 
In Pferdekliniken ist Personal oft mit MRSA nasal kolonisiert (prädominant ist eine Tierklinik-spezifische Subpopulation von MRSA CC398), wobei regionale und lokale Unterschiede bestehen, die auch von der Compliance des Personals mit Basishygienemaßnahmen abhängen (z.B. konnte bei tiermedizinischen Fachangestellten die Kolonisationshäufigkeit durch Einhaltung von Barriere-Maßnahmen (Mund-Nasen-Schutz; Handschuhe) signifikant vermindert werden).

Es wurde gezeigt, dass durch eine Verbesserung der peri-operativen Prophylaxe beim Modelltier Pferd Antibiotikagaben deutlich reduziert werden können (Antibiotic Stewardship), ohne dass es zu einer klinischen Verschlechterung der Tiere kommt. Die Mikrobiota dieser Pferde zeigt eine intestinale Dysbiose sowie initial einen weiteren Verlust an Biodiversität, welcher assoziiert ist mit einer Zunahme der Häufigkeit einer Kolonisierung mit MRE

Im Kleintierbereich wurden Therapieempfehlungen für Harnwegs- und Hautinfektionen von Hunden und Katzen auf der Basis prospektiver klinischer und mikrobiologischer Daten entwickelt, um den Einsatz von Antibiotika zu reduzieren und optimieren. Außerdem wurden individuelle Dekolonisierungsprotokolle für mit PVL-positiven S. aureus besiedelte Haustiere erarbeitet.
